Consent to Treatment
We want to do good for as many people as possible.
We believe people are good, but sometimes the law can be misused.
To protect everyone, and in line with standard medical practice, we require a standard treatment consent (similar to what you sign at any doctor's office) and in-person pickup with photo ID.
If you have no questions about your vaccine, you can complete the consent online in advance to speed up your prescription pickup at our next pop-up.
Please submit at least 24 hours before the event to streamline the process. You can also sign your consent at the event.
